Permanent Mission of the Union of Myanmar to the United Nations in New York is pleased to announce that following services will be rendered to Myanmar Citizens:

Renewal of Passport
Requirements
* Passport original and photo copy
* Up to date receipt of income tax
* Renewal fees $ 167 (cash,money order and Bank cheque only) Note : Myanmar Passport holders are required to renew their Passports before the date of expiry. A fine will be levied if the Passport is not renewed in time.
Issuance of New Passport
Requirements
* Application letter
* Copy of all pages of the Passport
* Income Tax payment must be up to date when applying for a new Passport
* Two passport-sized photos
* Personal history form ( C.V. form )
* Fees $ 310 (cash, money order and Bank cheque only )
Note : Myanmar Passports holders and their dependents are eligible to apply for a new Passport when there are only a few pages left.
Renewal of Certificate of Identity ( C of I )
Requirements
* Application letter by parents / guardian
* Up to date income tax receipt of parents/ guardian
* Photo copy of C of I
* Renewal fees $ 84 (cash,money order and Bank cheque only )
Issuance of New Certificate of Identity ( C of I )
Requirements
* Application letter by parents / guardian
* Up to date income tax receipt of parents/ guardian
* Copy of current C of I ( if applicable )
* Personal history form ( C.V . form )
* 2 photographs
* Issuance fees S$ 167 (cash, money order and Bank cheque only)
Attestation of Powers of Attorney
Requirements
* Revenue stamped document.
* Signature of 2 witnesses.
* Particulars of witnesses such as Passport No. NRC No, address must be stated.
* Person authorizing GP / SP must come to the Permanent Mission in person and sign before the Consular officer (Required to bring an original passport to be seen).
* Photo copy of GP / SP and passport
Attestation of Certification
Documents such as Birth Certificate / Marriage Certificate/ Degrees / Diploma / Driving license..... Etc. need to be notarized by a registered Notary Public in Myanmar and; subsequently endorsed by the Ministry of Foreign Affairs in Nay Pyi Taw, before submitting to the Permanent Mission for attestation / certification.
For First Time Payment of Income Tax
Requirements
* Company letter stating employees' monthly basic salary.
* Application Letter addressed to the Permanent Mission.
* One Photograph
* Personal History Form ( C.V. form )
Loss / Damage of passport
Requirements
* Application letter stating detail of loss / damage
* Police report ( for lost only )
* Copy of passport
* Personal history form ( C.V. form )
* Two photographs
* Payment of fine
Matters needed to be referred to the respective Ministry
* Inclusion of new born child in mother's Passport
* Change of status
* Separation of child from mother's Passport and issuance of new Certificate of Identity ( C of I )
* Inclusion of a child who holds C of I to mother's Passport
* Change of status to dependent of foreign husband or wife
* Change of status from seaman to shore job and vice versa
Legalization of Document
Original document(s) that is (are) to be legalized must be submitted together with the followings:
* Letters issued from Notary Public
* Request letter addressed to Myanmar Mission to legalize the document
* A set of copy of document(s) that to be legalized.
Service charge is $ 15 per document.
Opening Bank Account(s) at or Changing Signatories of Bank Account maintained with one of the Banks in Myanmar
Original document(s) must be submitted together with the followings:
* Request letter addressed to Myanmar Mission
* A set of copy of document(s).
After submitting the document, appointment date will be informed to sign the document in the presence of an officer from Permanent Mission. NRIC card of person who will sign on the Bank Specimen Card must be brought along on the appointment day. Service charge is $ 10 per document.
